The Coordination and Counselling Team in the County of Funen - A Model for Regional Organisation for Rehabilitation and Counselling
What is the Team?
The Coordination and Counselling Team in the County of Funen is a brain injury council, as recommended by the National Board of Health, but it is also much more than that. It is a secretariat opened 5 days a week from 8 to 4 based upon interdisciplinary teamwork.
The presentation will include the following topics from the interdisciplinary teamwork: The target group, needs of the target group and strategies to meet these needs, the structure, the organisation, the budget, the functions, the staff and long term objectives.
The coordination and counselling team has the following main functions:
- Coordination of provisions for brain injured persons transversing public sectors.
- Referral in relation to the county’ social provisions.
- Establishing coherent rehabilitation plans.
- Counselling functions in relation to the municipal authorities, the municipal institutions and to the institutions within the county domain.
- Explanation and information functions in relation to users, user-organizations, relatives and the general public.
Why a coordination and counselling team?
As a result of the actual conditions experienced.
The brain injured themselves, their families, as well as the professionals in the system expressed a growing frustration in connection with the totally lacking overview and co-ordination of the existing offers which were/and are available. This deficiency arises even more when transversing from the county sector to the municipal sector.
Coordination of the different disbursement systems can give considerable problems in themselves.
In addition, existing offers and proposals have been created for different target groups, which means that often the involved personnel have only a superficial knowledge of the special problem of the brain injured. The result is a situation which does not make it easier for the patient or the personnel.
